ManageMyHealth is an online system that allows you to access your medical records and view medical conditions, lab results, immunisation records, allergies, prescriptions and share health information as required with other healthcare providers. It offers services such as booking online appointments, secure messaging, ordering repeat prescriptions and requesting email consultations. ManageMyHealth would help you improve health outcomes by sharing your health information with healthcare providers and important individuals in your life.

You can come into the practice to register for ManageMyHealth or fill in the form below and email to firstname.lastname@example.org.
Shared care record
Your health information is now accessible by more clinicians involved in your care. Although we have decided as a practice to implement the shared care record system any other health professional treating you must still ask your permission to access your shared care record if you visit another GP, an after hours medical centre or a hospital emergency department. If you don’t wish other health providers to have access to your shared care record you can fully opt-out at any time by talking at us at the centre or phoning the shared care record project line on 0800 727 664, or sending a letter to the freepost address below:
