We are now enrolling new patients

We are now accepting new patient enrolments.

Whilst we are working under level 4 lockdown, the Ministry of Health have confirmed that a person does not have to be present to enrol with the practice. The enrolment form can be done over the phone or downloaded and completed at home then scanned back to us. A signature and ID are still required but these can be dealt with once we emerge from level 4.

Download and print the form. Fill in at home then scan and email to us on administration@hv.radiusmedical.co.nz. If you do not have a scanner at home, you could take a photo and email it to us – the photo must be clear and legible.
